Start date: Immediate start, Salary: £20.29 per hour, 3 days remote – 2 days per office

FTE Social Worker – base Bassetlaw area – Monday to Friday 8.30am – 5pm finishing at 4.30pm on a Friday. Assessment visits are critical – car driving required. SWE registered.

The Urgent and Emergency Care Teams follow the Discharge to Assess Model. Discharge to assess supports people to leave hospital when it is safe and appropriate to do so. Our teams work as part of an integrated multidisciplinary transfer of care hub based within the hospitals to plan supported discharges with short‑term social care support as needed. Once discharged, the goal is for the person to continue their recovery, rehabilitation and reablement, where we will then support with assessments to promote the individual’s independence and well‑being.

Please note the location of this vacancy will be Bassetlaw Hospital (S81 0BD) and Retford Hospital (DN22 7XF) and therefore there will be an expectation that you cover all sites. There will be an expectation to be on site x 2 days per week.

Urgent & Emergency Care Team (UEC NORTH) – Based at Bassetlaw Hospital and Retford Hospital.


Key Responsibilities

* Maintain a complex caseload and be responsible for the assessment, support planning and review of complex individual needs and then initiate and co‑ordinate a range of outcomes to meet them that promote independence.
* Undertake assessments using a range of health and social care tools, for example, contributing to continuing healthcare assessments.
* Be responsible for the identification of potential Reablement opportunities and provide access to those services as required.
* Working in accordance with the Nottinghamshire practice framework, to plan and lead inclusive assessments of strengths and resilience as well as vulnerabilities, developmental needs and risks.
* Compliance with legislation and departmental policies/procedures.
* Maintaining timely, accurate and clear written records, writing reports and other documents as needed and, maintaining the child’s record within the Department’s electronic recording system.
* Work effectively with children, their families and colleagues from a range of different agencies to plan, coordinate and review outcome focused plans.
* To model a strengths‑based approach to practice.
* Participation in team activities – team meetings, case discussion, review of team work.
* Participation in regular supervision.
* Contributing as appropriate to practice and service development.
* To take up opportunities for relevant training and development, in accordance with the requirement to evidence continuous professional development.


The successful candidate will also be expected to:

* Work in accordance with the Nottinghamshire County Council Code of Conduct and Social Work England’s (SWE) code of practice.
* Work within Departmental Policies and Procedures.
* Demonstrate Continual Professional Development in line with SWE’s regulations and requirements.
* Offer mentoring support to less experienced workers.
* Take up opportunities for further Professional Development which may include:
o Further Post Qualifying Awards (Higher Specialist and Advanced Awards)
o Practice teaching Key Accountabilities


Health and Safety

* You will take reasonable care for your health and safety and that of other persons who may be affected by the performance of your duties and where appropriate to safeguard the health and safety of all persons and premises under your control and guidance in accordance with the provisions of Health & Safety legislation, and Authority and Departmental Codes of Practice and Procedures.
* You will exercise proper care in handling, operating and safeguarding any equipment, vehicle or appliance provided, used or issued by the County Council or provided or issued by a third party for individual or collective use in the performance of your duties.
* You will personally implement and positively promote equal opportunities in service delivery and employment practices.
* Within resource constraints, you will promote and deliver fair and quality services that are sensitive and responsive to customers.
* You will work with Council provided technology, and associated systems, as required. You will personally ensure compliance with the County Council’s GDPR, Freedom of Information Act and ICT codes of practice.
* In order to achieve the objectives of the role, the post holder will need to work flexibly, including out of hours when required.
