Band 3 Childrens Community Carer

An excitingopportunity has arisen to provide clinical care to children/young people withadditional health needs (including tracheostomies and ventilators) at home
1. Minimum of hours per week
2. Flexible working to include night shifts 10pm-7am, weekends andbank holidays
3. You will need access to a car with business insurance to travel tothe family homes within Surrey
4. Previous experience is desirable however not essential as fullcompetency training is provided; there will be an initial induction periodfollowed by supervised practice

Purpose
6. To assist in the delivery of care activities in the child/young persons own home and in the community following an intensive training programme and competency assessment. Staff will work unsupported following assessment with the support of Clinical Case Manager. Participates in the induction and development of new staff, under the direction of the Clinical Case Manager.
7. To be flexible to work unsocial hours, including night duty over a 24 hour period, 4/5 week rota pattern including weekends and Bank Holidays
Principal duties to include:
Communicationskills (factor 1)
8. Establishes and maintains effective verbal andwritten communication with colleagues, children/young people, families andmembers of the multidisciplinary team, providing clinical information asrequired. There may be barriers tounderstanding.
9. Acknowledges the boundaries of the role,
10. MaintainsProfessional working boundaries with the families and colleagues at all time
11. Raises any issues/concerns with the Clinical CaseManager relating to the childs clinical care, workload and responsibilities.
12. Uses work mobile telephone and work emailaccounts appropriately, maintaining regular contact with the team andresponding in a timely manner.
13. Uses the team On Call system appropriately outof hours.
Analytical and judgemental skills(f3)
14. Responsible for identifyingchanges in patients conditions and taking appropriate action, documenting andreporting to Clinical Case Manager.
Planningand organisational skills (f4)
15. Effectively identifies allclinical equipment and disposables are in place to ensure safe delivery ofpatient care
16. Any shortfall or concerns shouldbe documented and reported to the core team or out of hours where appropriate
Physical Skills (f5)
17. Ability to use keyboard and computer
18. Ability to complete competency workbooks including tracheostomy,ventilator and gastrostomy care
19. Required to develop skills to change oftracheostomy breathing tubes, replacement of Percutaneous EndoscopicGastrostomy (PEG) feeding tubes etc.
20. Required to have developed skills in the use ofvarious equipment that requires accuracy within a narrow margin for error ventilators, oxygen saturation, enteral/nasogastric/nasojejunal feeding pumpsetc.
Patient / client care (f6)
21. Responsible for working with children/youngpeople who are dependent on others for meeting some or all of their health andwellbeing needs.
22. Ensure the care that is delivered to thechildren on the caseload is of a consistently high standard
23. Responsible for discussing the childs/youngpersons support plan with the qualified nursing team, ensuring understandingof their role in providing care to meet the needs, including an understandingof the Safeguarding of Children.
24. Responsible for delivering care in accordancewith relevant care plans, escalation plans, Standard Operating Procedures andguidance.
25. Required to administer medication followingMedicines Management Training and having been deemed competent by a ClinicalCase Manager.
26. Responsible for caring for highlytechnologically dependent children/young people.
27. Responsible for assessing the child/youngpersons condition in relation to the child/young persons support care planand escalation plan.
28. Ensure any concerns relating to the child/youngpersons clinical condition, safety or in relation to safeguarding concerns areescalated and reported.
29. Understands service issues to enable problems tobe identified and solutions recommended.
30. Able to prioritise work to meet child/youngpersons care needs.

Essential
41. Good general education and literate in English and numerate
42. Level 2 NVQ or equivalent in health related subject
Desirable
43. NNEB
44. NVQ level 3 in Childcare, Care or equivalent working with children
Experience

45. Experience of working with children or caring for children in a health, education or social setting.
46. Understanding of a range of work procedures and practices, some of which are non-routine, which require a base level of theoretical knowledge.
