Overview
Position Summary: 30-hour Social Worker needed for the Boston Children’s Hospital Primary Care Center in Weymouth
Responsibilities
* Providing clinical social work services to assist patients and families with personal, family, medical and environmental difficulties.
* Performing psychosocial and risk assessments, assigning diagnoses and developing and implementing interventions and treatment plans.
* Participating in planning and implementation of discharge and outpatient service plans and collaborating with health care team and community agencies to plan
* Implementing and following up on treatment and provision of needed services.
Minimum Qualifications
* Education: Master’s Degree in Social Work
* Certification/Licensure: LICSW required
Experience
* Flexibility and ability to work in fast-paced multidisciplinary team environment as well as excellent communication skills.
* Experience with social services agencies, understanding of social determinants of health and trauma ideal.
* Bilingual/bicultural/diverse candidates are encouraged to apply. Spanish speaking a plus!
Compensation
New hires may be eligible for a $3750 sign on bonus
