Are you an experienced children’s nurse looking for a new challenge?

The role will be based in Oxford but you will be required to work countywide.

Main duties of the job

As a Children's Community Nurse, you will work autonomously to manage a caseload of patients. You will safeguard and advocate for the child, facilitate their care and provide training so that their health needs are safely managed at home and in school.

The Children’s Community Nursing Team are expected to deliver respite sessions and support the duty role, acute (including Hospital at Home) and palliative care rotas.

The acute/Hospital at Home aspect of the role includes undertaking planned clinical assessments and interventions following care pathways ensuring any deteriorations are appropriately escalated. You will be working closely with the doctors in the acute setting to support care of these children at home where appropriate. The service operates a single point of access for families or agencies to contact; the nurse in charge prioritises and allocates nursing tasks accordingly to the localities.

We offer end of life care at home, in order to support this, we operate an on-call rota which is activated when required. Training and support will be provided for this role.

The Children’s Community Nursing Team offer a rolling education programme, including a full induction for new starters into the team. Detailed job description and main responsibilities

The Oxfordshire Community Children's Nursing team provide vital support to children from birth to 18 years with complex nursing needs across the county. The children on our caseload have a variety of chronic and acute conditions and require nursing support such as enteral feeding, long term ventilation, symptom management and support with acute childhood illnesses. The support we provide includes empowering families to care for their child's health needs at home, providing respite sessions, delivering acute and palliative care outside of the hospital environment, and supporting children at school. We are commissioned to provide all of these services within each of our four localities.

This role is based in Oxford, however we offer a countywide service and travel to other areas/bases is required on a regular basis. Applicants must have a full valid driving licence and have access to a car for work.

Our core service hours are between 08.00-20.00, 7 days a week, working contracted hours as a shift pattern, including weekends and bank holidays. The on call palliative rota operates for 24 hours from 08:00– 08:00, this is currently covered by Band 6 staff and above. If necessary, flexible working adjustments can be considered in line with the Equality Act 2010.

13. Palliative Care Certificate

Knowledge & Experience

Essential criteria

14. Acute paediatric nursing experience
15. Knowledge and understanding of safeguarding and promoting the welfare of children, young people and vulnerable adults
16. Excellent communication skills
17. Evidence of clinical leadership

Desirable criteria

18. Community Nursing experience
19. Paediatric nursing skills in palliative care
20. Paediatric nursing skills in Intravenous Therapy administration, Cardio-respiratory care, enteral feeding and wound care
21. Experience in acting as a student nursing supervisor/ assessor

Job related skills

22. Awareness and acceptance of the rights and needs of service users and staff
23. Awareness and acceptance of Children’s Rights
24. Ability to work in different environments
25. Full UK driving licence and access to car
