What you will do: - Lead as the clinical expert within CICC and ESD for stroke and related conditions involving communication or swallow disorders. - Conduct specialist assessments, diagnoses, and management for patients with communication and swallowing difficulties post-stroke and from other medical conditions. - Mentor rehabilitation assistants and less experienced speech and language therapists. - Stay updated with innovative assessment and treatment practices, encouraging the team to adopt new methods. - Develop and deliver training programmes for the Dysphagia Trained Nurse programme. - Communicate effectively with healthcare professionals to coordinate patient care. - Prioritise and manage your own clinical caseload independently. - Use clinical assessments to facilitate patient consent discussions. - Implement individual care plans following best practices. - Evaluate clinical effectiveness using outcome measures like Care Aims. - Customize patient interactions, considering cultural, educational, and physical needs. - Ensure smooth transitions of patient care and maintain accurate case notes in line with professional standards.
