Team Leader - Peer Support and Social Prescribing (Daytime)
You will be based at the new 24/7 Neighbourhood Mental Health Hub at Acomb Garth Community Care Centre, with potential to work from other Neighbourhood Mental Health Hubs in the city (Clarence Street and Tang Hall) and York Mind Office (Highcliffe House, Clifton). This permanent role is subject to funding and involves line management of peer supporters and social prescribers within a multidisciplinary team, contributing to the development of community mental health hubs and peer support and social prescribing services at York Mind. The role supports the service manager with data collection and reporting feedback.
Hours and location
We are recruiting 3 posts: 1x 36 hours (3x 12 hour shifts per week) and 2x 24 hours (2x 12 hour shifts per week). You may indicate a preferred pattern within your application.

Main duties
* To work within the Community MH Hub and be part of the multidisciplinary team.
* To provide effective line management to peer supporters and social prescribers, as part of a matrix management structure.
* To offer client facing work as required.
* To proactively support the day-to-day coordination of the peer support and social prescribing service alongside the Peer Support and Social Prescribing Manager, working in accordance with processes and procedures.
* Maintain accurate case notes and play an active role in gathering feedback and data to support reporting requirements of the service.
