As a qualified Physical Health Nurse, you will lead and coordinate all aspects of the physical health needs of service users, while also supporting your colleagues and promoting good teamwork with the multidisciplinary team, such as GPs and other physical health departments.
You will be responsible for monitoring observations and assessments to identify any actions that need to be addressed and documented.
As a Physical Healthcare Nurse, you will be:
* Adhering to the NMC Code of Conduct and facilitating compliance with local, Trust, and national standards of practice.
* Developing and supporting physical health interventions used in the clinical area, based on current evidence-based practice.
* Engaging actively in practice development, evaluation, and audit activities relevant to physical healthcare and/or service areas to promote evidence-based practice.
* Working collaboratively with primary health services, developing and maintaining links with GP practices and other physical health departments.
* Monitoring and maintaining Physical Health Dashboards.

Where you will be working:
Castleholme Lodge is situated in Huddersfield with excellent transport links. It is undergoing a complete refurbishment to provide the best environment for service users and staff.
Castleholme Lodge will be a 20-bed care home with nursing, offering mental health services for adults with severe and enduring mental health difficulties. It will be part of a cluster of Elysium services across Yorkshire and the Midlands that work closely together to support each other.
