An exciting opportunity to join the Community mental health - Older adult service, an expanding team working with patients with both dementia and functional disorders.
The MDT team is made up of Consultant Psychiatrists, Nurses, OTs, Social workers, Psychologists, Assistant practitioners and support workers.
The Band 5 CPN will take on the key worker role, working predominantly with patient presenting with memory difficulties, however, as the team grows and develops may include some work with patients presenting with functional illness also.
The team is very supportive, with a focus on development, therefore the role would be suitable to a newly qualified nurse. Preceptorship will be offered and opportunities to undergo further training as your career progresses.
Main duties include;
Care planning using Dialog and Dialog+
Risk assessments, including positive risk taking.
Undertaking of medication monitoring and annual reviews.
Completion of assessment and treatment/ intervention plans for patients with an existing dementia diagnosis and older adult patients.
Psychoeducation with patients and carers.
Duty on a rota basis.
Support of group work.
Post diagnostic support, including signposting individuals and/ or carers to community resources.
