The Complex Needs Team is a mental health team thatsupport people with co-occurring mental health & substance use issues. Theteam provide assessment of needs, development, implementation and evaluation ofcare for patients from a specified group of patients who have a diagnosis of asevere and enduring mental health condition with co-occurring substance useissues. In addition to complex needs, high risk behaviours' and difficultieswith engaging with services.

The service provides a Plymouth city wide service. TheMental Health Practitioner role provide a holistic assessment, evidenced basedinterventions and assertive support for people with co-occurring mental health& substance use issues.

The post holder will work alongside the otherservices within the Plymouth Alliance that support clients that experiencemulti disadvantage and live complex lives within the community. The aim of thework the Complex Needs Team undertake is to promote recovery from dependentsubstance use and improve the health and wellbeing outcomes for individuals,linking them in with community activities and wider support networks.

The rolewill work across the Plymouth Alliance in a variety of settings, in homelessprovisions, community setting and alongside VCSE partners. There is anexpectation that staff may be required to work in other parts of theorganisation/complex lives system on request according to service need and aspart of personal development.


Main duties of the job

The post holder will carry acaseload of clients with co-occurring mental health and substance use issues.

To provide of line management& caseload supervision to Band 5 & below.

To contribute to professionaldevelopment, appraisal, performance review and recruitment.

The post holder will act as aCare-Coordinator for a defined group of patients and organise care inaccordance with the Personalised Care Framework.

To contribute to professionaldevelopment, appraisal, performance review and recruitment.

The post holder will ensurethat their clinical practice is based on evidence and good practice and inaccordance with any NICE guidance.

The post holder will beindividually accountable for their practice in accordance with the NMC, HPC,Social Work England professional code of conduct.

To promote positive risk toallow clients independence and autonomy of choice.

To liaise with othercommunity resources and facilitate use of mainstream resources.

To maintain a caseload ofclients with a significant degree of mental health complexity.

To participate in the shiftlead and provide appropriate supervision.

This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.

Job responsibilities

Practice within the legal andethical framework set out by mental health related legislation to ensure thatservice users interests and well-being are at the fore.

Liaise, advise, develop andmaintain good relationships with service users, carers, relatives and fellowhealth, social care professionals who are involved in the delivery of services.

Engage in support and careplans for those who have complex needs, high risk behaviours alongside dynamicrisk assessments and effective management of risk.

Work collaboratively with AdultSocial Care to identify and implement any eligible Care Act needs.

Participate in all activitiesaimed at keeping professional skills updated and contribute to dissemination ofthat knowledge and experience to all staff and students within your area ofresponsibility.

Act upon referrals fromsecondary mental health services, other primary care workers consultants andother statutory bodies by providing triage, assessment, implementation anddevelopment of programs of care to individuals, groups and families, who are difficultto engage in mental health services.

Plan, with teachingestablishments, a realistic learning experience for students and forpostgraduate students and be responsible for the quality of the program.

Participate in and supporttraining programs designed for all staff groups.

Monitor standards of practicewithin the team, ensuring areas of deficit are addressed through audit.

Co-ordinate the collation ofevidence within the team in support of standards for better health improvingquality.

Prioritise and re-assign teamworkload and develop caseload management system.

Assist the Team Manager inthe Investigation of complaints made regarding the service.

Actively support the teamthrough change in organisation and service delivery.

The post holder will attend and lead communitydrop in clinics in the homeless provisions across the city.


Person Specification


Knowledge & Skills

* Skilled and experienced clinician, specifically in assessment, management of risk & crisis intervention.
* Good communication skills, including verbal and written skills.
* Empathic outlook on health care delivery which respects clients dignity and human rights.
* Good interpersonal skills with a range of stakeholders.
* Supervisory skills.
* Calm and adaptable, with the ability to work flexibly when required.
* Have the ability to work autonomously and part of a MDT.
* Recognise and manage stress in self and others.
* Have the ability to commute around the locality.
* To work creatively, be enthusiastic and self-motivated.
* Have a strong ability to manage change & support both patients and professionals with transitions.
* Demonstrate a willingness to participate in service development.
* Have the ability to dynamically to support the Team Manager in developing a healthy team culture, and driving forward team improvements and change at all times.
* Specialist clinical skills e.g: CBT training, Psycho-social Interventions, DBT Skills.
* Staff appraisal skills.
* Knowledge and skills in medication management relevant to the post.


Qualifications

* - GCSE Grade A to C in English & Maths.
* - Must hold a relevant Undergraduate or Postgraduate degree in:
* - Nursing
* - Social Work
* - Occupational Therapy
* and a professional registration with one of the following:
* - NMC - HCPC - Social Work England
* Further professional development in;
* - Recovery CBT
* - Solution Focus Therapy
* - Dialectical Behavioural Therapy
* - Motivational Interviewing


Experience

* Relevant experience of working with adults who use substances in problematic & harmful ways.
* Relevant experience in a Community Mental Health setting or clear evidence of how candidate is able to fulfil the functions of a Community Mental Health Practitioner.
* Care co-ordination experience.
* Case load management, and managing a complexity of mental health and substance use risk.
* Experience at a senior level which includes managerial experience, staff supervision and operational issues.
