Overview

South & West Herefordshire Primary Care Network is looking for a Care Coordinator to join our Health & Wellbeing Team. The position will work with our Frailty and Neighbourhood Health teams to support the PCN in offering access to proactive and reactive personalised care and support, with a particular focus on supporting those patients at highest risk of suffering from health inequalities.


Responsibilities

* To work closely with the PCN and practice teams to identify and work with a cohort of people to support their personalised care requirements, using any available decision support tools.
* To act as the primary point of liaison for the PCNs multi-disciplinary team meetings, capturing action steps at all meetings, and ensuring that all agreed activity is completed.
* To contact patients identified for outreach to arrange for appointments and to promote healthy lifestyle choices, such as smoking cessation and health checks.
* To support the members of the Frailty and Neighbourhood Health teams with outreach to identified cohorts of patients.
