The role

This is an exciting opportunity for an experienced Physiotherapist within Renfrewshire HSCP.

This post is set within a multidisciplinary team working across acute and community, supporting those with frailty to maximise their independence and wellbeing within their local communities, supporting early discharge and preventing avoidable hospital admissions.

We welcome applications from experienced and innovative practitioners with an appetite for service improvement and self-development. This post will work across the primary-secondary interface, and join the HFRS team aligned within acute front doors in Renfrewshire and Glasgow City HSCP areas. Critical to the success of these roles will be facilitating and enabling communication between and within multi-disciplinary teams across acute and community services.


Duration, Location, and Working Pattern

* This is a fixed-term or secondment post of 1 year
* The post is full-time, although part-time options may be considered for a period of 1 year to provide resilience to the service expanding to cover 7 days. This is being offered as either a fixed-term or a secondment opportunity, and as such, applicants for secondment require agreement from their current line manager prior to application.
* Your main based will be Renfrewshire House


Key responsibilities

You will work autonomously across a diverse range of health and social care settings, taking on not only your professional clinical responsibilities but also case coordination, health promotion, audit activity, staff and student supervision and induction, and providing expert advice on professional interventions.

You will manage a complex clinical caseload within the service, applying your specialist knowledge and advanced skills to deliver high-quality care, while working as an effective and collaborative member of the interdisciplinary team. A key part of your role will be supporting patients following discharge from hospital and helping to prevent inappropriate readmissions by ensuring safe and effective care within the community.

You will provide specialist, direct clinical care, including comprehensive assessment, treatment, and ongoing management of patients referred to the service. Working in partnership with professionals across health, education, and social care, you will deliver coordinated and person‑centred care for adults with a wide range of clinical conditions.

In addition, you will support and supervise less experienced colleagues, including other professionals, support workers, and students, while also offering guidance and expert advice to members of the interdisciplinary team and external agencies regarding appropriate interventions.

You will work independently to manage a delegated caseload, carrying out assessments, developing and implementing care plans, and evaluating outcomes to ensure the best possible patient care. Your role will also involve supporting the assessment and ongoing management of patients within the community and at the acute interface, helping to avoid unnecessary hospital admissions.


Knowledge, training, qualifications and/or experience required to do the job

* Degree, Diploma, or equivalent qualification in Physiotherapy
* Registered with the relevant Health Professions Council (e.g. HCPC)
* Excellent communication skills
* Driving licence
