Job Responsibilities
* Act as an autonomous practitioner in accordance with and upholding the NMC Code of Conduct and Scope of Professional Practice for Nurses, Midwives and Health Visitors.
* Assess bladder and bowel function, physical and psychosocial needs, plan, manage, implement and evaluate individual programmes of patient care in an efficient and clinically effective way, meeting patient and carer needs. Provide education on correct application of continence aids and offer informed choices for lifestyle modifications, treatment options and alternative products.
* Use a range of assessment tools, such as a portable bladder scanner, to inform assessment.
* Work mainly unsupervised to assess, plan, implement and evaluate bladder and bowel pathways of patients, providing skilled nursing care in patients' homes or other appropriate settings.
* Prioritise a delegated caseload/workload and manage time effectively within the team.
* Work in close liaison with nursing colleagues, members of the multidisciplinary team, social care and voluntary sector.
* Role‑model the values expected in Compassion in Practice (2014) and ensure patient, family and carer feedback supports compassionate care.
* Record information objectively and report back to the Bladder & Bowel Service.
* Plan ahead for potential scenarios to ensure anticipatory care needs are understood and met.
* Recognise signs of deterioration in patients and refer appropriately to ensure safety and avoid hospital admission.
* Collaborate effectively with other members of the multidisciplinary team or other agencies involved in patient care.
* Participate in case conferences, MDTs and care package reviews, providing health‑related information when requested.
* Make accurate observations of patients’ physical, mental and social condition, communicate findings to the Bladder & Bowel Service via the Coordinator and other members of the Primary Healthcare Team, and judge problems requiring investigation, analysis or samples.
* Check, administer and dispose of drugs according to Trust policies and guidelines.
* Provide support and education in the delivery of care, delegate work appropriately to junior team members, and undertake staff appraisals when appropriate.
* Participate in the practical instruction of students.
* Encourage and support patient independence and self‑care of their condition while minimising risk for both patients and staff and using all available resources.
* Takes reasonable precaution to safeguard patients and their property.
* Assess clinical and environmental risk and share with appropriate services.
* Promote UTI improvement work and antibiotic stewardship.
* Serve as the named mentor for pre‑registration students.
* Promote the health and well‑being of clients and their families/carers through health promotion activities.
* Take part in the prevention and control of the spread of diseases.
* Undertake joint visits with specialist nurses, GPs, social workers and other health professionals.
* Order appropriate products via home delivery service after patient assessment.
* Ensure that equipment loaned no longer required is returned to the supplier.
* Report clinical incidents and successful interventions through the Datix system as part of a safety culture.
* Participate in audits and encourage completion of patient experience questionnaires.
* Implement policies and suggest changes to practice and procedures.
* Maintain good working relationships with colleagues and other agencies.
* Communicate effectively with patients, carers, relatives and other professionals.
* Support continence promotion projects and raise public awareness of continence issues.
* Deliver agreed educational programmes to other practitioners in group or one‑to‑one settings.
Person Specification
Qualifications
* Registered General Nurse (RGN)
* Current NMC registration
* Evidence of further recent professional development
Experience & Knowledge
* Understanding of national government initiatives in health and social care and how they impact local services
* Knowledge of a broad range of bladder and bowel conditions, including LTCs such as neurological conditions, stroke, dementia and other common illnesses, frailty, and palliative care
* Understanding of presentations of multiple pathologies, depression and anxiety states and frailty, predominantly in older people
* Knowledge of managing uncomplicated symptoms in patients with bladder and bowel problems and enhanced communication skills to confidently manage uncertainty
* Understands the role of the Continence Nurse in the community
Desirable
* Mentorship module or working towards it
* Community nursing experience
