Responsibilities
* Assess, plan, implement and evaluate care of the child/young person in community setting, children's centre and acute paediatric ward. Cater to four main groups: acute and short term conditions, long term conditions, disabilities & complex conditions, life limiting & threatening conditions.
* Facilitate safe and effective care in partnership with CYP, family/caregivers and multidisciplinary team, ensuring holistic and family‑centred care, supported by senior community children's nurse.
* Provide nursing care within child's home or educational setting, including gastrostomy management, non‑invasive ventilation support, wound care, injections, intravenous therapies, oncology blood sampling, line care and other clinically appropriate interventions.
* Attend MDT meetings, discharge planning, TAF and CP meetings, provide condition‑specific training to families and education staff.
* Contribute to palliative and end‑of‑life care in home; work with senior team for 24‑hour service when required.
Qualifications and Experience
* Experience in paediatric/community nursing.
* Ability to independently manage a designated caseload with multidisciplinary team support.
* Strong clinical decision‑making, evidence‑based practice.
