We are looking for an enthusiastic and motivated practice nurse to join our experienced team.
Working part time or full time, flexibly between Mon-Fri 0800-1800, the ideal candidate will have had primary care experience, although we would provide training and mentorship to the right candidate.
The successful candidate will be a excellent communicator and team player.
Main duties of the job
Responsible for the delivery of high-quality practice nursing care to our patient population of 10,700, the Practice Nurse will:
1. deliver evidence-based care to those presenting in our urgent care clinic
2. Have a good understanding of the management of people with chronic health conditions.
3. Have excellent communication skills to assess patients holistic needs and establish relevant cultural, social and economic factors in a non-judgemental manner.
4. Work both autonomously and as part of a team.
5. Be able to demonstrate care and clinical decision-making skills for the management of patients with complex medical needs.
6. Prioritise and triage the needs of patients and recognise when to seek further support from the duty GP.
7. Have a good understanding of preventative care and be able to offer advice on lifestyle choices.
About us
Our practice team provides general medical services to our 10,700 patients in a close-knitcommunity. Llandrindod Wells has beenvoted one of the happiest places to live in Wales and we are in a greatlocation for enjoying the great outdoors.
From out modern single-site surgery, our 10 GPs, experienced practice nurses, ANP, paramedic, HCAs, pharmacy support, MSK physiotherapist and visiting specialist nurses are supported by a full admin team.
We are a training practice for medical, PA and nursing students and are looking forward to our first GP registrarin August. We also have a GP Fellowjoining the practice.
We work hard and enjoy a good social life with trips to the rugby in Cardiff and meals out.

Job Responsibilities:
Diagnosing and treating patients presenting with minor illness
15. Triage and treat patients wishing to see a health care professional.
16. Ensure clinical practice is safe and effective and remains within boundaries of own competence.
17. Advise and treat patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary.

Chronic Disease Management:
26. Contribute to the management of chronic diseases, such as hypertension, diabetes, asthma, COPD, anticoagulation, AF/CHD, etc.
27. Have a good understanding of the nature and management of chronic diseases (in accordance with current NICE guidelines and practice policies and procedures).
28. As part of a team, he/she will run chronic disease management clinics autonomously, whilst working within their own competencies.
29. Appropriate training to be provided where necessary.
Pathological specimens and investigatory procedures
30. Undertake the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures requested by the GPs
Administration and professional responsibilities
31. Participate in the administrative and professional responsibilities of the Practice team
32. Ensure accurate notes of all consultations and treatments are recorded on the patients computer records
33. Ensure the clinical computer system is kept up to date with accurate details recorded and amended
34. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice
35. Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit
36. Attend and participate in Practice meetings as required
37. Restocking and maintenance of clinical areas and consulting rooms
Training and personal development
38. Training requirements will be monitored by yearly appraisal and will be in accordance with Practice requirements. Personal development will be encouraged and supported by the Practice. It is the individuals responsibility to remain up to date with recent developments.
39. Participate in the education and training of students of all disciplines and the introduction of all members of the Practice staff where appropriate.
40. Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.
41. Undertake all mandatory training including e-learning in accordance with practice and statutory requirements.
42. Develop and maintain a Personal Development Plan and professional portfolio to bring to annual appraisal.
43. Maintain subscription to NMC and revalidation.
Liaison
44. As well as the nursing team, there is a need to work closely with reception and office staff to ensure the smooth running of the Practice.
45. There is also the need to establish and maintain good liaison with other surgeries and agencies including secondary care
Meetings and team working
46. It will be necessary to attend and contribute to various Practice meetings (regular clinical meetings and nursing team meetings) as requested. The only reason for not attending will be annual, study or sick leave.
47. To ensure relevant information is disseminated to and from the nursing team.
48. Share/cascade learning outcomes as appropriate.
49. Help develop and maintain Practice policies and protocols for all nursing activity
50. Ensure agreed Practice audits are carried out
51. Ensure the Practice Manager informed of any matters arising or problems / potential problems
