Offer description

This is an exciting opportunity to join our team during a time of real change and growth. We are recruiting four full‑time Community Care Officers (CCOs) across our South and East Mental Health teams. The 12‑month fixed‑term roles have the potential to become permanent at the end of the contract period.

As a Community Care Officer you will play a vital role in supporting adults with care and support needs to live as independently as possible, helping to shape how we deliver high‑quality, strengths‑based mental health support across the county.


Key responsibilities

* Manage a diverse caseload of non‑complex and complex mental health cases.
* Complete strengths‑based Adult Needs Assessments and Reviews in line with the Care Act.
* Build effective, therapeutic relationships with individuals, families and carers.
* Support people to define and work towards meaningful, recovery‑focused outcomes.
* Promote independence, resilience and community inclusion.
* Identify and address barriers that impact on wellbeing and progression.
* Work in partnership with health, housing and voluntary sector colleagues.
* Respond flexibly to service demands, including unplanned visits.


Working conditions

* Core office hours: 08:45–17:15 (Monday–Thursday), 08:45–16:45 (Friday).
* Hybrid working model: a combination of home and office‑based working, with office attendance as required.
* Travel is essential; mileage is reimbursed for journeys from the office to appointments.
