Overview
We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Support Worker, who is keen to deliver a range of quality care services to patients visited by the Care Community Team based at Chowley Oak Tattenhall. The team visits patients of all the Rural Alliance Care Community Care Team. This post is part time - 30 hours per week. The Trust currently supports the Real Living Wage (RLW) hourly rate as set by the Living Wage Federation and will apply a pay supplement if the national A4C hourly rate falls below this. This post involves some lone working and providing care to those in the final days/weeks of life, supporting patients to prevent acute hospital admission and facilitating discharge from hospital. The post requires travel across the Western Cheshire footprint; access to transport for work purposes is essential.
Responsibilities
* Provide care in patients’ homes including tasks such as simple dressings, catheter care, personal care and recording blood sugars.
* Deliver end of life care and support to patients and families in a compassionate and respectful manner.
* Work as part of a friendly team while sometimes working independently (lone working).
* Support patients to prevent acute hospital admission and facilitate discharge from hospital.
* Travel across the Western Cheshire footprint; access to transport for work purposes is essential.
Qualifications and experience
* Appropriate related qualification - or willingness to complete an apprenticeship appropriate to the Clinical Support Worker role.
* Completion of or working towards an apprenticeship and gaining qualification equivalent to level 3 or above in a subject relevant to the role of Healthcare Assistant.
* Demonstrate a range of work procedures and practices and exhibit knowledge and abilities.
* Completion of or commitment to complete the Care Certificate (CWP In-house 2-day training to be completed within 3 months of start date); understanding of need for confidentiality, empathy and sensitivity.
* Desirable: Basic wound management.
* Desirable: Basic risk assessments and record keeping; ability to demonstrate good quality care while adhering to Trust policies and procedures, and working towards completing Care Certificate.
* Experience of working collaboratively with colleagues, wider teams, service users, etc.
* Experience of supporting others in a care setting (professional or lived experience).
* Desirable: Basic clinical skills including venepuncture, skin care, wound care, clinical observations.
* Experience of working in a community care setting including provision of personal care.
