Senior Community Staff Nurse (Carlisle Healthcare)

Carlisle Healthcare District Nursing Team are an emerging and unique team that put people at the heart of what we do. We are looking to appoint two motivated and enthusiastic Senior Community Staff Nurses to work as part of our dedicated Community Nursing team and the wider Integrated Care Council (ICC). This is a development opportunity for those seeking the next step in their career, involving holistic, patient‑centred care and complex caseload management.


Main duties of the job

You will be supported by the District Nursing Team Lead and Band 6 District/Community Nurses to deliver nursing care covering weekdays, weekends, bank holidays and evenings. Responsibilities include:

* Providing high‑quality community nursing services to patients with complex needs.
* Working with primary health care teams and other health and social care professionals to deliver coordinated care.
* Managing and leading the clinical caseload for the community nurse team.
* Coordinating patient discharge and encouraging independence to prevent hospital admission.
* Collaborating with partners and stakeholders across rural and urban areas.
* Ensuring evidence of meeting all essential criteria is documented.


Qualifications

* Level 1 registered nursing qualification.
* Evidence of continuous professional development to degree level or equivalent experience.
* Post‑registration qualification in acute and chronic disease management.
* Nurse prescribing ability.
* Specialist qualification in community nursing or equivalent.


Experience

* Ability to use initiative and prioritise effectively.
* Demonstrated teamwork and the ability to motivate peers and manage change.
* Strong problem‑solving skills and critical decision making.
* Leadership experience and operational clinical management of a team.
* Knowledge of the changing health agenda and its impact on community nursing.
* Recent community nursing experience.


Knowledge

* Understanding of the extent of the role and the scope of professional practice.
* Broad knowledge base of acute and chronic conditions.
* Appreciation of reflective practice and its application.
* Self‑awareness and recognition of personal limitations.
* Knowledge of how health services are planned, delivered and evolving.
* Awareness of current changes in healthcare delivery and impact on community services.


Skills and Aptitudes

* Excellent communication and interpersonal skills.
* Ability to maintain confidentiality.
* Enthusiasm for working as part of a team.
* Post‑registration clinical skills.


Personal Circumstances

* Able to travel independently.
* Able to work 24/7 and flexibly across a range of in‑patient and community settings.


Other requirements

* Ability to promote equality and value diversity (level 3).


Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and requires a submission for Disclosure to the Disclosure and Barring Service (formerly CRB) to check for any previous criminal convictions. The DBS cost will be met by the successful candidate via salary deduction.
