We have an exciting opportunity to join our dedicated D2A team, helping people regain independence in the comfort of their own homes. This is an opportunity to join a team making a real difference to people's lives in your area.

About the Role:

The D2A (Discharge to Assess) service provides short-term, high-quality support to individuals who are well enough not to need an acute hospital bed but do need some extra support in their home. You will ….

* Assess customers’ needs to identify areas where support is needed to promote independent living.
* Motivate customers to build their confidence so they can achieve the goals they have set and return to independent living.
* Support customers with tasks around the home and with personal care such as personal hygiene, toileting, and dressing.
* Work a mixture of early and late shifts between 7 am and 10 pm and alternate weekends.

About you:

A passionate and experienced individual, you will have a background in care and will hold your QCF Level 2 in Health and Social Care. You’ll be passionate about supporting people to be as independent as possible.

Alongside this you will also need a full driving licence and access to your own vehicle as you will need to travel between clients.

Your future team:

You’ll join our specialist D2A team who cover the areas of Bridgwater, Highbridge, Burnham-On-Sea, Cheddar and surrounding areas. The team works with clients who have been discharged from hospital, supporting them to recover in the place they call home.
