Overview
As an Apprentice Patient Pathway Support, you will play a key role in ensuring the smooth running of clinics by preparing and tracking clinic documentation, and supporting the SFHT Patient Pathway and Patient Pathway Coordinators. You will help maintain a high standard of service across both King’s Mill and Newark Hospitals, contributing directly to the Trust's goals. This is an exciting opportunity to join our dedicated Urology Department at King’s Mill Hospital. We are seeking a self-motivated, innovative individual with excellent attention to detail to support the delivery of high-quality patient care.

Responsibilities
* Make appropriate preparations for clinics, to ensure that the patient's visit will run smoothly.
* Prompt tracking in and out of case notes using the PAS system to maintain accuracy and completeness of all electronic records in accordance with Data Accreditation requirements.
* Provision of typing support, as required, to support Pathway Coordinators.
* Maintenance of good working relationships with the Patient Pathway Coordinators, liaising and raising issues as necessary and building good relationships with patients and other members of staff.
* Ensuring adherence to the Trust's confidentiality policy.
* Deal with telephone enquiries from patients, relatives, clinicians and other Trust staff in relation to outpatient appointments and clinics.
* Deal with cancellation of clinic appointments and rebooking of patients and bring patients appointments forward when required to support Patient Pathway Co-ordinator.
