Summary
We are seeking a Care Coordinator/HR to join our team in [RM203LH]. Your efforts will directly support both service users and their families in navigating the care process.
Responsibilities
* Coordinate and manage care plans for service users
* Liaise with families and healthcare professionals to assess individual needs
* Ensure compliance with Care Quality Commission standards
* Monitor the delivery of care services and make adjustments as necessary
* Support service users in expressing their preferences for care
* Provide guidance and support to staff members involved in care delivery
* Maintain accurate records and documentation related to service users
* Promote a positive and respectful environment for all service users.
* Qualifications
* Proven experience in a healthcare or social care setting (high)
* Strong organizational and communication skills (high)
* Ability to work collaboratively with families and healthcare providers (somewhat)
* Office experience (somewhat)
* Knowledge of Care Quality Commission regulations (somewhat)
* Compassionate approach towards service users (high)
