Barnsley Primary Care Network is looking to recruit a Pharmacy Technician todevelop medicines optimisation services and become apart of the primary care skill mix.

This is a newrole and will develop over time, but the expectation is that you will work aspart of the multi-disciplinary team to support member practices with medicinesoptimisation. As this is an evolvingdevelopment position we will support the successful candidates to develop theirrole within primary care - which will includes a full induction programme,ongoing training, a rotation programme across medicines optimisation workstreams and supervision both within the PCN. You will work alongside experiencedclinical pharmacists and technicians in the Medicines Management Team atBarnsley CCG across practices in Barnsley. This role requires the candidate to be fully committed toundertaking the 15 18 month CPPE Primary Care Pharmacy Education Pathwaythis is an essential requirement for this post.


Main duties of the job

The successful pharmacytechnician candidate will play an important role in complementing our clinicalpharmacists, other members of the PCN multi-disciplinary team and the medicinesmanagement team. This is an evolving role with huge scope for developmentas we expand our PCN team further over the years.

Candidates will have excellentwritten and verbal communication skills, experience of influencing others, andworking within professional boundaries.

Its an essential requirementfor candidates to be registered the General Pharmacy Council as a pharmacytechnician with a BTEC or NVQ3 in Pharmaceutical Sciences or equivalent.

About us

Barnsley has established onesuper Primary Care Network (PCN) of over 250,000 patients which includes allof GP practices across Barnsley. This is supported by six NeighbourhoodNetworks building on our successful and existing model of neighbourhoodworking; this enables us to maintain and focus on the specific needs of eachlocal area whilst allowing integration at a borough wide level.

Barnsleys primary care pharmacyteam is a fully integrated model providing excellent opportunities forprofessional development and building strong links across the system.


Job responsibilities

JOBPURPOSE:

To support the PCN practices towards thedelivery of the medicines Optimisation QIPP agenda and other prescribingobjectives.

To provide medicinesoptimisation advice and support to GPs, other practice staff, ICB colleagues.

To deliver training to GPpractice staff to support the implementation of medicines optimisation workstreams.

Develop medicinesoptimisation services across the network working alongside experienced primarycare clinical pharmacists and technicians and to become a part of the primarycare skill mix.

To undertake medicines optimisationreviews, audits, medication changes and other medicines optimisation relatedactivities in selected Barnsley PCN practices and care homes in line withpractice, care home and PCN policies.

To assist with the monitoring andanalysis of prescribing data at PCN and practice level and offer support topractices which are outlying on key prescribing indicators.

PRIMARYDUTIES AND AREAS OF RESPONSIBILITIES

CLINICAL

Undertake patient facingand patient supporting roles to ensure effective medicines use, through shareddecision-making conversations with patients.

Carry out medicinesoptimisation tasks including effective medicine administration (e.g. checkinginhaler technique), supporting medication reviews and medicines reconciliation.Where required, utilise consultation skills to work in partnership with patientsto ensure they use their medicines effectively.

Support medicationreviews and medicines reconciliation for new care home patients and synchronisemedicines for patient transfers between care settings, linking with localcommunity pharmacies.

Support the ClinicalPharmacist in Structured Medication Reviews (SMR) including organisingnecessary monitoring tests prior to SMR.

Provide expertise toaddress both the public health and social care needs of patients, includinglifestyle advice, service information, and help in tackling local healthinequalities.

Manage shared careprotocols and liaise with Clinical Pharmacists for more complex patients.

Support initiatives forantimicrobial stewardship to reduce inappropriate antibiotic prescribing.

Support National andlocal safety agenda and deliver on behalf of the practice.

TECHNICAL AND ADMINISTRATION

Support the PCN todeliver on the Barnsley medicines optimisation scheme, QIPP, QOF and locallycommissioned enhanced services

Support the PCNmulti-disciplinary team to ensure efficient medicines optimisation processesare being followed.

Implement efficientordering and return processes and reduce medication wastage.

Provide training andsupport on the legal, safe and secure handling of medicines, including theoperation of the Electronic Prescription Service (EPS).

Promote the safe andappropriate implementation of Electronic Repeat Dispensing (eRD) and onlineordering.

Develop relationshipswith other pharmacy professionals and members of the multi-disciplinary team tosupport integration across health and social care including primary care,community pharmacy, care homes, secondary care and mental health.

Support theimplementation of national prescribing policies and guidance within GPpractices, care homes and other primary care settings.

Support practicereception teams in handling general prescription requests, in order to allowGPs and clinical pharmacists to review the more clinically complex requests.

Support the PCN inreviewing and developing practice policies for CQC requirements.

INFORMATION SUPPORT AND RECORD KEEPING

To keep an accuraterecord of all activity undertaken within practices.

To ensure all patientinterventions are recorded on the practice computer in accordance with theworking procedures developed with the practice.

To be responsible forkeeping accurate retrievable records of all formal queries, data collectionexercises, and audits.

TRAINING AND EDUCATION

To supervise and trainpractice receptionists undertaking the generation of repeat prescriptions andto explain and inform them of any prescribing changes being carried out.

To providing training topractice clinical/administration staff on any medicines management issues asagreed by the practice.

Be willing to undertake any training requiredfor the role e.g. the CPPE Primary care pharmacy education pathway.

Attend Barnsley placeMedicine Optimisation Team meetings and peer groups.

Attend CCG MedicineOptimisation Team training and development sessions.

Liaise with colleaguesincluding the Barnsley place Medicines Optimisation Team on prescribing mattersto ensure consistency of patient care and benefit.

Participate in theBarnsley place Medicines Optimisation Team medicines related projects andworkstreams.

Liaise and integrate withcolleagues in the Barnsley place Medicines Optimisation Team to benefit frompeer support.

Work with primary care toensure practices are compliant with CQC standards where medicines are involved.

To support public healthcampaigns and to provide specialist knowledge on all public health programmesavailable to the general public.

KEY RELATIONSHIPS

Liaise with other stakeholders as needed forthe collective benefit of patients, key relationships include (but are notlimited to):

Patients and theirrepresentatives.

GPs, nurses and otherpractice staff.

Social prescribers, firstcontact physiotherapists, physician associates and paramedics.

Accountable to theAccountable Clinical Director of Barnsley PCN and to the Clinical Directors ofthe Neighbourhood Networks.

Person Specification


Experience

* Up to date clinical knowledge of drugs and procedures and familiarity with key prescribing information resources.
* Knowledge of administrative procedures, project management or information analysis.
* Working knowledge of Microsoft Office including Word and Excel.
* Experience of working with repeat prescribing systems.
* Previous primary care medicines management experience.
* Working knowledge of GP clinical systems.
* Awareness of a range of Health Services provisions.
* Knowledge of NHS issues


Qualifications

* BTEC or NVQ3 in Pharmaceutical Sciences or equivalent with induction experience and relevant work experience AND current registration with the General Pharmaceutical Council.
* Willing and committed to undertaking the CPPE Primary Care Pharmacy Education Pathway
