Responsibilities
* Provide specialist Occupational Therapy assessment, treatment & review of patients with advanced palliative conditions or complex deteriorating neurology, often in an unstable or deteriorating stage of their condition.
* Work as a Band 6 Occupational Therapist within the Community Specialist Palliative Care Team, as part of an integrated interdisciplinary team including Occupational Therapists, Physiotherapists and Therapy Technicians.
* Deliver a comprehensive, evidence‑based range of assessment and treatment to adults in Coventry with specialist palliative needs, in the patient’s own home or other community setting.
* Follow defined referral criteria for the Specialist Palliative Care Team.
* Liaise closely with team members and other community services to support patients’ effective therapy.
* Provide advice and guidance to patients, carers and staff working in health and social care.
* Support the professional development of staff within other services where appropriate.
* Contribute toward service development and meeting key performance indicators.
* Assist in quality and activity monitoring systems, supporting the evaluation of effectiveness of the service.
* Adhere to the lone worker policy.
* Utilise specialist skill and knowledge to manage a palliative care and neurological caseload, ensuring the physical, psychological, behavioural, perceptual, cognitive, sensory and environmental needs of patients are addressed.
* Adopt a patient‑centred and sensitive approach to therapy, collaborating with patients and/or carers in all aspects of the therapy process (assessment, planning, intervention, evaluation, reporting).
* Maintain a high level of knowledge of a variety of conditions including neurological, orthopaedic, musculoskeletal, care of the elderly, respiratory, and falls, understanding the impact of multi‑pathological rapidly changing presentations.
* Utilise a comprehensive range of evidence‑based specialist assessments and treatments to patients in their own home or in a clinic environment.

* Provide an immunisation and vaccination report from either your current UK occupational health provider or your UK GP.
