We invite high calibre applicants to embrace this exciting opportunity to join the dynamic and enthusiastic multidisciplinary team at the Aberdeen City Hospital at Home service as we continue with planned expansion, in the role of Advanced Nurse Practitioner/Advanced Clinical Practitioner.
Hospital at Home provides short duration episodes of high quality, hospital-level acute care, to patients within their own homes, avoiding or reducing duration of hospital admissions, and supporting the wider service to improve healthcare capacity, resilience and patient outcomes. You will be flexible to the needs of the role and service, and will support the Leadership Team in developing the service to assure safe, patient-centred and effective care delivery, applying the 4 pillars of practice at an advanced level.
