Health Care Assistant

Main duties of the job
1. Be part of an MDT team receiving clinical and managerial support, aligned with social care professionals in accordance with the NHS 10-year plan.
2. Deliver quality care to a designated caseload in patients' homes, following care plans devised by community nursing staff.
3. Provide concise feedback during daily handovers to ensure needs are met and risks are escalated.

Job responsibilities
The successful candidate should be flexible to assist colleagues across the wider team, with access to a car essential for service duties. A full induction will be provided. The posts are based in Mid Bedfordshire, serving communities in Ampthill, Shefford, Biggleswade, Sandy, Potton, Flitwick, Barton, and Woburn. The service operates between 08:00 and 21:00, with rotational shifts including weekends, evenings, and Bank Holidays. Part-time options are available.
Person Specification
Education/Qualifications (Essential)
* NVQ Level 2 or significant care experience, with willingness to work towards NVQ Level 3
Desirable
* NVQ Level 3, recent Care Certificate, GCSE or equivalent
Knowledge and Skills (Essential)
* IT literacy, good verbal and written communication
Desirable
* Rehabilitation skills, data collection, high-quality care delivery, safeguarding, risk management
Other Requirements (Essential)
* Manual handling skills, current driving license, ability to travel, fluent English communication
Experience (Essential)
* Experience in Community Health or Social Care
