* Maintain accurate records of client interactions, care plans, and service delivery in compliance with organisational policies.
* Collaborate with multidisciplinary teams to enhance the quality of care provided to clients.
* Provide support and guidance to clients and families navigating the healthcare system.
Experience
* Previous office experience is essential for this role, demonstrating strong organisational skills and attention to detail.
* Experience in a healthcare setting or familiarity with care coordination processes is highly desirable.
* Excellent communication skills, both verbal and written, are required to effectively liaise with clients, families, and healthcare professionals.
