CJP Outreach Service is currently recruiting Support Workers to join our team as we prepare to welcome a fourth client to our service. Experience in Positive Behaviour Support and supporting individuals with learning disabilities, autism, and behaviours of concern is advantageous. Ideally, candidates will have experience in community-based settings such as supported living or residential care.
As a support worker, you will play a pivotal role in providing person-centred support for adults with complex behavioural needs and communication difficulties, helping them achieve their goals. You will support individuals in developing essential life skills and independence.

Responsibilities include:
* Providing person-centred support.
* Collaborating with individuals to create and implement tailored support plans.
* Implementing Positive Behaviour Support (PBS) strategies to improve quality of life and reduce behaviours of concern.
* Supporting the development of life skills such as cooking, personal hygiene, budgeting, and household management.
* Assisting with medication, medical appointments, and maintaining health and well-being.
* Supporting community engagement and social activities.
* Maintaining accurate care records, documenting progress and incidents.
* Working closely with colleagues, families, external professionals, and agencies to ensure holistic care.
Working hours
Support is provided on a 24/7 basis with staffing ratios of 1:1 and 2:1 support. Shifts are rotational, including 12-hour and 15-hour shifts, every other weekend, with sleep-in duties available.
