Role Overview
For community nursing colleagues within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board, this role offers an opportunity to deliver whole‑person care to housebound patients. You will lead the clinical team to allocate caseloads, triage referrals, and coordinate care safely, effectively, and according to plan.
Responsibilities
* Lead the senior clinician role: triage new referrals, assess complex patients, and plan their care needs.
* Coordinate the multi‑disciplinary team: allocate work to members with appropriate competencies and supervise junior staff and students.
* Maintain sound clinical skills and effective risk assessment to keep patients safe.
* Deliver evidence‑based care, adhering to current policies and procedures.
* Engage patients and carers in partnership, respecting, empowering, and working collaboratively.
* Act as a role model, supervising junior members and contributing to the team’s success.
Qualifications & Skills
* Band 6 Health Care professional (HCP) – expected to be kind, responsive, and professional, contributing to quality service outcomes.
* Experience in community or housebound nursing, with proven clinical expertise and team leadership.
* Strong communication and decision‑making skills for complex patient assessment.
* Full UK driving licence (mandatory) – if you cannot drive due to disability, contact the Recruitment Team to support you.
