About the Role
As an Assertive Outreach Support Worker, you'll:
* Provide intensive, person-centred support to individuals in temporary accommodation such as B&Bs, hostels, and night shelters.
* Reach out to those rough sleeping, helping them access the services and support they need.
* Carry out needs and risk assessments, developing tailored support plans with each person.
* Work closely with partner agencies (e.g. housing, health, police, and social services) to ensure a joined-up approach.
* Help people to build support networks and prevent tenancy breakdowns.
* Be part of a wider team committed to ending homelessness in Monmouthshire.
* This role is varied, challenging, and rewarding - no two days are the same, but every day you'll be making a positive impact.
About You
We're looking for someone with:
* At least 1 year's experience supporting people affected by homelessness or in a related support setting.
* Strong communication and relationship-building skills, with the ability to engage people who may be mistrustful of services.
* The ability to work effectively with multiple agencies and partners.
* Good IT and organisational skills.
* A full driving licence and access to a vehicle (essential for this role).
* You'll be empathetic, resilient, and able to stay calm under pressure. Most importantly, you'll share our commitment to giving people the tools they need to rebuild their lives.
