Overview
The Patient Pathway Co‑ordinator is a pivotal role at the heart of patient care, ensuring every individual receives a seamless, timely and compassionate experience from referral through to discharge. Acting as the central point of contact for patients and supporting an allocated Consultant, this role delivers outstanding service within elective care pathways.
Key Responsibilities
* Manage appointments, waiting lists, and RTT targets, safeguarding timely access to treatment while maintaining exceptional data quality standards.
* Coordinate clinics and theatre schedules, arrange pre‑operative assessments, and oversee correspondence.
* Ensure every stage of the pathway runs efficiently and smoothly.
* Build trusted relationships with patients, clinicians, and support teams to contribute to service excellence, performance achievement and continuous improvement.
