Our client is a campaigning organisation and a multi-project service provider working across West London, supporting disabled people, those experiencing other disadvantages and their families to live healthier, happier, and inclusive lives by ensuring their needs, rights and aspirations are met.
They are looking for a Care Coordinator to be based in their Head Office
Key duties:
* Provide exceptional customer service, accurate information, and support tailored to meet the staff and client needs.
* Ensure policies and procedures comply with CQC regulations and work to achieve the best care standards.
* Produce staff rotas on a monthly-basis and manage staff cover during absences.
* Participate in the out of hours emergency on-call rota.
* Conduct and maintain client risk assessments and care plans.
Our ideal candidate:
* Qualification in Health & Social Care – Minimum of NVQ level 2 or 3.
* Excellent communications and writing skills.
* Caring, compassionate and understanding towards people in need of care and support.
* Self-motivated and keen to learn.
