What You Will Do:
* Conduct comprehensive assessments to identify the needs of individuals experiencing substance misuse challenges and develop tailored recovery plans.
* Provide structured one-to-one key working sessions and facilitate group programmes to support sustained recovery.
* Collaborate with healthcare providers, social services, housing teams, and other agencies to deliver holistic, person-centred care.
* Maintain accurate, timely records and monitor client progress, reviewing and adapting support plans where necessary.
* Deliver education and harm reduction advice to clients and their families, promoting long-term positive outcomes.
