Overview
This is a brand-new role for an experienced Health Care Assistant to work both in the practice and to be part of a home visiting team visiting housebound patients and those in care homes. The successful applicant should have a clean driving licence and access to a car. Mileage will be paid.
Responsibilities
* Phlebotomy duties (taking blood samples)
* Taking blood pressure
* Performing ECGs
* Delivering NHS Health Checks and helping with diabetic checks
* Undertaking basic wound dressings
* Supporting and carrying out diabetic checks/NHS Health Checks
* Assist the home visiting team visiting patients that are housebound and in care homes
* Liaise with and communicate effectively with other members of the Primary Health Care Team

Job Description
Responsibilities
Principal responsibilities include phlebotomy duties, taking and, where appropriate, dispatching laboratory specimens (blood, urine, swabs and other diagnostic tests) as required by GPs. Ensure specimens are ready for collection by the designated collection time or stored in the fridge for next-day collection. Set up the ECG machine to take ECGs and ensure they have been analysed by a GP. Carry out blood pressure checks. Support the nursing team in diabetic clinics, CHD clinics, healthy eating and weight management, flu clinics, and coil/implant clinics. Undertake basic wound care and suture/clip removal, assisting the nurses. Undertake NHS Health checks and administer flu and B12 injections having been adequately trained. Ensure fridge temperatures are regularly monitored. Maintain high standards of tidiness, hygiene and safety in the use of all equipment and areas of work, in line with Practice Health and Safety, Infection Control and other Practice policies. Undertake any additional duties as required, including working across sites. Liaise with and communicate effectively with other Primary Health Care Team members. Attend courses and study days as required to maintain high standards and document them as part of continuing professional development. Assist the home visiting team with housebound and care home visits. Any other duties as specified by the Partners in line with training and experience.
