Job summary
We are looking to recruit a highly motivated Practice Nurse to join our friendly Team. The ideal candidate would have experience of working in General Practice.
Main duties of the job
The prospective candidate should have experience in chronic disease case management, including asthma, diabetes, CHD also INR star/warfarin clinics, cervical screening, childhood immunizations, wound care, cardiac monitoring and ECG's, travel immunizations, minor illness.
About us
We are a single-handed dispensing practice based in Leeswood which is 3 miles from Mold and approximately 10 miles from Wrexham. We dispense 98% of our registered patients medication to them rather than them having to take a script to the chemist. This works well for our patients as the bus service to the village is not the best.
The Team consists of a Practice Manager, 2 x receptionists, 4 dispensary staff, HCA, GP and a regular Locum GP. We are all a friendly bunch and ready to welcome a new member.

The prospective Practice Nurse will hold a Registered General Nurse qualification and have an active registration with the NMC, have excellent communication skills with an ability to prioritise your workload and be a Team player ready to assist your work colleagues if required.
Experience in chronic disease case management including, asthma, COPD, Diabetes, CHD and INR star. Also experience in the following nursing procedures; cervical cytology, childhood immunizations, cardiac monitoring and ECGs, travel immunizations, minor illness, wound care and phlebotomy.
Hours of works can be negotiated between - pm Monday to Friday and some flexibility is required.

1. Experience of working within a primary care setting as a Practice Nurse.
2. Experience of chronic disease management including asthma, CHD, Diabetes, Hypertension.
3. Experience of carrying out ECG's, venapuncture, cervical cytology, wound care, travel medicine, womens health, mens health, childhood immunizations.
4. Requesting pathology tests and processing the results, advising
5. patients accordingly
6. New patient medicals

8. Understands the importance of evidence-based practice
9. Broad knowledge of clinical governance

11. Ability to work within own scope of practice and to understand
