An exciting opportunity has arisen within our Continuing Health Care Team. We are looking for highly motivated and experienced RMNs, RGNs OR Allied Health Professionals who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centered assessments so CHC and FNC eligibility can be determined. Roles involve assessing patients for CHC, holding a caseload, and reviewing patients, as necessary.
Job Description
Job responsibilities
The post holder will be responsible for undertaking Specialist Health Needs assessments for individuals who are undergoing assessment for, or who are already eligible for NHS Continuing Healthcare and NHS Funded Nursing Care.
The post holder will be required to work with the individual and/or their representative, and to liaise with private providers, partner organisations, along with other health and social care professionals to ensure that all assessments are person centred.
The post holder will be required to work flexibly to support the Individualised Commissioning team by participating in the \'Duty\' rota, supporting with any urgent referrals or any safeguarding investigations.
Key working relationships
* Work closely with patients, residents, carers, colleagues in Local Authorities, Acute Trust, Independent Sectors, and neighbouring localities and any other relevant groups.
Partnership working / collaboration
* Develop and maintain effective working relationships in order to ensure continuity of care and address identified patient needs.
* Liaison with a variety of providers arranging assessment of patients in order that the most appropriate placement/ care provision is identified.
* Work with neighbouring localities, social services and service providers to ensure that joint commissioning arrangements achieve the best quality services for the local population.
* Work collaboratively with independent sector providers to ensure that any commissioned services are quality assured, monitored and supported.
Strategy, policy, service development and implementation
* Ensure monitoring tools are implemented according to CHC procedures.
* Assist in the development of policies and procedures, as requested, which will support the overall delivery of CHC/ FNC.
* To support and contribute to the ongoing development of the service identifying areas of practice that require improvement.
Responsibilities for Physical and Financial Resources
* Negotiating care costs for specialist placements outside of agreed framework for Nursing and Homecare providers ensuring quality and value for money.
* Monitoring of packages of patients in receipt of CHC and FNC.
* Contribute to the effective monitoring and use of the CHC budget.
* Ensure that all commissioned care is effective, person centred and value for money.
* Demonstrate both quality of care and value for money in all placements/ commissioned care.
Communication
* To communicate with patients and their carers regarding their health needs and appropriate care provision.
* Develop effective communication networks with Nursing Home Managers and partner agencies.
* To communicate effectively with all team members and Senior Management colleagues.
* To develop effective relationships with colleagues from partner agencies.
* To communicate clearly and succinctly using written, verbal and electronic methods.
* Receive sensitive or contentious information relating to patients, families or carers regarding their physical and mental health needs.
* Communicate sensitive information to families and carers in relation to a patient needs and eligibility.
* To have overall responsibility for the maintenance of accurate records, relating to each case.
* Maintain Broadcare database recording specific data in line with patient confidentiality and data protection.
Planning and organising
* Work with neighbouring localities, Social Services and service providers to ensure that joint commissioning arrangements achieve the best quality services for the local population.
* Facilitate meetings, including case conferences, best interest meetings and discharge planning meetings to ensure continuity of care across agencies.
* Organise own workload to deliver timely, comprehensive assessments, care planning, care package implementation, ongoing reviews and case management.
Patient Care / Responsibilities
* To be responsible for assessing patient needs as defined in the National Framework for NHS Continuing Healthcare and Funded Nursing Care.
* To lead and exercise professional judgement in the achievement of an accurate determination or assessment of patients needs.
* To provide clinical support and advice to Care Homes and Home Care Providers following identification of skill gaps in relation to the care of the patient
* To support patients who are eligible for CHC and wish to access a Personal Health Budget.
* Work within the Mental Capacity Act and the Care Act.
* Meet professional responsibilities under current Safeguarding Vulnerable Adult policy and procedures.
* Ensure compliance with and updating all patient records on the Broadcare database, keeping accurate individual electronic records.
* Manage individual cases ensuring that healthcare is provided to meet assessed needs.
* Ensure that any change in patients assessed need is communicated appropriately to other professionals and carers.
* To undertake retrospective reviews, reviewing evidence and completing comprehensive Care Need Portrayals.
* Maintain up to date records as per NMC requirements ensuring data governance requirements are met.
* Be responsible for the Case Management of a designated caseload.
Information / data analysis, management and reporting
* Development of high standards of IT skills and competencies.
* Maintain a formal record of all cases on the CHC Broadcare database.
Leadership / management and HR
* Ensure adequate clinical care is provided with a requirement that all incidents, which may compromise the care needs of a patient are reported immediately to a senior manager.
* To make decisions and work within a framework commensurate with their level of knowledge and competence and to act accordingly with their sphere of responsibility.
* Responsible for the contribution to staff inductions and clinical supervision to professional colleagues as appropriate.
* Understand personal responsibility for respecting and promoting issues of equality, diversity and rights in accordance with good practice and legislation.
* Responsible for participation in the agreed appraisal process, identifying own/ others mandatory professional, personal development and training needs.

* Autonomously manage a caseload of individuals with highly complex health needs.
* Perform role in line with the NMC Code: Professional standards of practice.
* Respect confidentiality and maintain data privacy in line with organisational policies.

Required Qualifications and Experience
Essential
* First level Registered Nurse (Adult) with current active NMC registration (without restrictions)
* Academic/ professional qualification at degree level or equivalent level of experience
Knowledge
* A good level of knowledge of the National Framework for NHS Continuing Health Care Funded Nursing Care
* An understanding of the Mental Capacity Act 2005 and its application
* Good working knowledge of adult safeguarding
* Evidence of continuous professional development
Experience
* Significant post registration experience
* Experience of working in a multidisciplinary environment
* Experience of undertaking comprehensive holistic Nursing assessments
* Experience of care planning and Case Management with patients who have significant health needs
Skills
* Excellent written and verbal communication including providing and receiving complex, sensitive or contentious information
* Excellent report writing skills
* Excellent time management and prioritisation
* IT skills for inputting data and reporting
* Able to travel locally, regionally and on occasion, nationally
