Job overview

To assess, diagnose, plan and develop specialised rehabilitation programmes for patients referred to the Active Programmes team.

To supervise/train inexperienced nurses/assistants/students working within the service.

Deliver individualised cardiovascular programmes of care (for example phase 2, Heart Failure Angina, AF) using in-community programmes, or by self-directed manuals (where appropriate) or digital solutions or home visits.

Deliver individualised pulmonary rehabilitation programmes following assessment to ensure continuity of service provision to all patients referred to Active Programmes.

To use continuous assessment to evaluate treatment outcomes and adjust treatment plans accordingly, appropriately referring to other agencies to meet patients’ needs and promote integration of services.

Main duties of the job

1. Assess, plan, implement and evaluate individualised programmes of care.
2. To receive and communicate complex, sensitive or contentious information to patients and carers, who may have barriers to understanding eg dysphasic, complex mental health problems, to maximise rehabilitation potential and to ensure understanding of conditions.
3. To have expert verbal and non-verbal communication skills of motivation, explanation and reassurance to agree and gain co-operation of patients and carers in their treatment programmes.
4. To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of rehabilitation and to ensure a consistent approach to patient care and treatment e.g. moving and handling.
5. To provide nursing advice, teaching and training to other members of the MDT regarding the management of patients and the role of nursing.
6. To train, supervise and performance manage junior staff, senior assistants and assistants and students in a clinical, professional and organisational capacity. This may include undertaking formal appraisal.
7. To assess capacity, gain valid informed consent and have the ability to work within a legal framework with patients who lack capacity to consent to treatment.
8. To be responsible for ensuring the effective selection and use of all treatment resources.

This vacancy is advertised on a fixed term contract basis for a period of 12 months due to the post being externally funded. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts taking into consideration the ongoing source of the external funding and the requirement for the post to be maintained.

Essential criteria

9. Registered General Nurse
10. Evidence of post basic education at Diploma/Degree level
11. Demonstrate commitment to continuous Professional Development
12. Evidence of relevant post graduate development in respiratory &/or cardiac care
13. Trained in Basic Life Support
14. To have completed the national practice assessor training or equivalent teaching and assessing qualification for those with ENB998.

Desirable criteria

15. Degree in health related subject
16. ENB qualification or equivalent
17. Immediate Life Support trained
18. Postgraduate Certificate

Experience

Essential criteria

19. Post-registration experience and experience in managing respiratory, cardiology /cardiac conditions
20. Experience of supervising / managing / developing Pre- registration nurses
21. Experience of Audit

Desirable criteria

22. Experience in delivering cardiac or pulmonary rehabilitation
23. Experience of working in community setting (pre or post registration)
24. Involvement in research
25. Experience or involvement in service development

46. Due to the requirement to undertake home visits, the post holder is required to be either a car driver with a valid driving licence
47. Motivation, enthusiasm, confidence in and commitment to nursing practice
48. Willingness to work flexibly including working outside of traditional normal working hours in accordance with service need.
