Skilled Nursing Activities:
* Complete assessments of patients for effective symptom management.
* Utilize nursing skills, communication, and emotional intelligence to assess problems and needs.
* Perform treatments and interventions according to the plan of care and physician’s orders.
* Provide education on disease progression and end-of-life needs.
* Instruct patients and caregivers based on the plan of care or updates thereto.
* Collaborate with patients, families, caregivers, and practitioners on care goals and treatment changes.
* Follow precautions outlined in the care plan.
* Offer emotional support during interactions with patients and caregivers.
* Conduct follow-up and collaborative calls as needed or requested.
Communication and Coordination:
* Answer or return calls promptly during shifts, within 5 minutes unless engaged in patient care.
* Provide real-time updates on visit times and patient status to supervisors.
* Update supervisors on changes like transfers or deaths.
* Contact physicians or APRNs for care plan modifications.
* Coordinate care transitions, including GIP transfers, per policy.
* Work with the team to determine appropriate responses to calls or visits.
* Arrange medications and supplies based on urgency and policy.
* Communicate accurately with staff, physicians, community resources, and vendors regarding patient needs.
* Document all interactions in EMR promptly, following protocols.
* Provide detailed reports to the IDG team after all visits, including death visits.
Weekday Coverage:
* Serve as a team member, managing resources and performing skilled nursing activities for assigned adult patient caseloads or assisting the team as directed.
* Assess patient and caregiver needs; manage symptoms; educate on disease-related care; participate in end-of-life care/support with the IDG.
* Perform skilled nursing activities as outlined above.
* Communicate and collaborate with the IDG team as directed.
* Provide shift reports as required.
Minimum Qualifications:
* Diploma or Bachelor’s degree in Nursing.
* At least two years of experience in acute care, oncology, or home health care.
* Strong skills in teaching, communication, assessment, and IV therapy within a multidisciplinary team framework.
Licenses/Certifications:
* Current RN license in Florida.
* CPR certification.
* Valid Florida driver’s license with an acceptable driving record and reliable transportation.
Physical Requirements/Work Environment:
This role involves 8-12 hour home care shifts, requiring use of personal vehicle. Reasonable accommodations are available for individuals with disabilities.
