Responsibilities
* Contribute to assessment, planning, implementation, and evaluation of patient care within set protocols and within the scope of the Assistant Practitioner remit.
* Undertake delegated clinical and non-clinical tasks requiring technical knowledge and judgement.
* Support staff in the development of clinical procedures, protocols and practice to ensure effective delivery of service to patients in the Trust.
* Undertake all relevant forms of care with minimal supervision and delegate work appropriately to other members of the team.
* Maintain high standards of professional behaviour, aligning practice with the Trust's values.
Holistic Needs Assessments
* Patient telephone calls
* Telephone calls prior to treatment summary appointments
* Telephone support calls with patients 10-14 days after diagnosis
* Telephone support calls with patients 7-10 days after 1st cycle of chemotherapy
* Referrals, for example to physiotherapy, counselling, lymphoedema clinic, Macmillan, orthotics for wigs, moving forward course, etc.
* Post op telephone support calls
* Caseload (which is overseen by CNS) of established Primary Endocrine Patients, whom will be seen in clinic and wellbeing telephone calls
* Assist Breast CNS in clinic, if required with post op patients and primary endocrine review patients
* Assisting with telephone enquires
* Telephone patients who are eligible for little lift boxes and complete referrals
* Undertake fitting clinic for Boost prothesis
* Monitor PEAKS and update information board
* Ward visits
* To be involved in education and health promotion
* Assist with audits
