Job summary
The Community Children's nursing team is part of an integrated network of services for children in Bexley and Greenwich, providing high-quality, child-centred care. The team of specialist and community nurses delivers highly skilled nursing care to children and families in their homes or community settings.
This role involves providing evidence-based nursing care for children and young people with conditions such as acute and long-term illnesses, disabilities, and complex needs, including palliative and end-of-life care. The post also requires offering leadership and clinical expertise to Band 5 Children's Community Nurses, ensuring safe, efficient, and evidence-based service delivery.
This position is suitable for an experienced children's nurse interested in developing specialised skills. The candidate should possess leadership skills and have experience in community or acute settings.
The post involves working weekends and bank holidays on a rotational basis and supporting 24-hour end-of-life care as needed.
Main duties of the job
1. Managing a caseload of children with diverse health conditions, including complex medical needs and short-term health issues requiring rapid intervention.
2. Supporting junior staff to develop skills and confidence in community nursing roles.
3. Collaborating closely with senior specialist nurses to support learning and professional development.

Job responsibilities
You will work within an integrated Children's Nursing team covering both Community Children's Nursing (CCN) and Hospital at Home (H@H). The role is varied, covering both boroughs of Greenwich and Bexley, with referrals involving children with complex medical needs, long-term conditions, and short-term needs requiring rapid response to facilitate early hospital discharge.

* Evidence of completion of Practice Assessor course (Essential)
* Children's Nursing Qualification (NMC registration at diploma or degree level) (Essential)
