About the Role
As a Social Worker in our Adult Social Care team, you'll play a crucial role in assessing and addressing the diverse needs of our community, navigating the varied challenges our vibrant city provides. Your responsibilities will include:
* Conducting assessments and developing tailored care plans for service users and carers, ensuring their unique needs and priorities are addressed.
* Advocating for choice and control, empowering individuals to make decisions and manage risks, ensuring services are organised in alignment with their preferences and needs.
* Upholding professional standards continuously as our service evolves. You'll monitor changing needs, develop support plans, utilise resources, and ensure Social Care remains future-ready through creative use of assistive technology.
