This position involves working closely with multidisciplinary teams to ensure safe, efficient, and patient-centred discharge planning. Key responsibilities include: Acting as the primary point of contact for patients, families, and healthcare professionals regarding discharge and care transitions. Assessing patient needs and developing tailored discharge plans that align with clinical recommendations and community resources. Liaising with internal teams and external agencies to facilitate timely referrals and continuity of care. Monitoring progress, addressing barriers to discharge, and escalating issues where necessary to maintain patient flow. Supporting quality improvement initiatives and compliance with hospital policies and national standards. This role requires excellent organisational skills, strong communication, and the ability to manage complex cases in a fast-paced environment. You will contribute significantly to improving patient outcomes and enhancing the overall experience of care. Maintain and update the Wardboard daily to ensure accurate visibility of patient progress and discharge priorities for the multidisciplinary team. Identify and address barriers to discharge, escalating issues where necessary to maintain patient flow and meet hospital targets. Arrange referrals and community support services, ensuring continuity of care post-discharge. Participate in board rounds and multidisciplinary meetings, providing updates and supporting decision-making. Contribute to quality improvement initiatives, ensuring compliance with hospital policies and national standards. Maintain accurate documentation and communicate effectively with all stakeholders to support patient-centred care.
