Main duties of the job
To provide Nursing services (see below) to the registered population of Bridge and Monkseaton Medical Practice, working as part of the Primary Care Team managing Long Term Condition Management and Health Promotion.
Working over 2 or 3 days this post will be ideal for a Registered Nurse looking to increase or decrease their hours. You will be expected to:
* Consult, with patients, perform the following nurse related duties as detailed below in the person specification
* give telephone advice, and undertake administration as is normally expected of a practice nurse.
* Take part in the in-house educational program.
* Contribute to the management a QOF and Enhanced Services
* Contribute to the new commissioning agenda on behalf of the practice e.g., NHS health Checks
* Ensure that stock management is kept within Practice formulary/budgets, set from time to time
* To work with the performance manager to deliver local and national targets

* Holistic diabetic care including management of risk factors and HBA1C
* Clinically managing results from patients who have NHS Health Checks and are high risk
* Good knowledge of infection control
* Pulse and Blood Pressure recording
* Childhood immunisations
* Management of acute illness
* Cervical screening smear taker
* Women's health and Contraception
Qualifications
* NMC registered Nurse
* Childhood immunisation
* Cervical screening smear taker
* LTC management
* Women's Health and Contraception
Experience
* Good team worker
* Willing to work across 2 practices if needed
