Responsibilities
* Work with other members of the team to assess patients' needs and risks.
* Provide evidence‑based care to a caseload of psychosocial patients, listening to and acting on their feedback so care is personalised.
* Deliver a program of psychosocial interventions, including information and advice on substance misuse, alcohol, smoking cessation, blood‑borne viruses and safe sex practices.
* Offer non‑judgemental care one‑to‑one and in group settings, supporting nursing and pharmacy teams in primary care within healthcare centres, on wings and house blocks.
* Implement the HiJ competency framework and policies, always considering safeguarding.

Qualities and Experience
* Evidence of CPD
* Relevant degree/qualification or equivalent experience
* Knowledge and experience in structured psychosocial interventions
* Experience working within a multidisciplinary substance‑misuse framework, including housing, social care, healthcare, and mental health pathways
* A non‑judgemental and compassionate approach
* Excellent communication skills
* Good IT skills and IT literacy

Annual salary up to £33,510 per annum (FTE), dependent on experience.
