Duties and Responsibilities
* Undertake patient-facing and patient-supporting roles
* Ensure effective medicines use through shared decision-making conversations
* Administer medicines, including checking inhaler technique
* Support medication reviews and medicines reconciliation
* Utilize consultation skills for partnership with patients
* Support Clinical Pharmacist in Structure Medication Reviews (SMR)
* Provide expertise for public health and social care needs
* Manage shared care protocols and liaise with Clinical Pharmacists
* Support the PCN to deliver on QOF, IIF, and other enhanced services.
