Resettlement Practitioner - Lincolnshire Action Trust
HMP Lincoln (LN2 4BD)
37 hours per week (applicants must be 18+ due to prison-based working)
Interview Dates: Telephone Interviews are expected to take place on 05/11/2025. Panel Interviews are expected to take place on 17/11/2025.

Responsibilities
* The successful person will use their skills and knowledge to determine levels of support required by individuals approaching release from HMP Lincoln, devise and carry out support plans, review progress, positively interact with other agencies and colleagues, be aware of risk and risk management, maintain case records and collect evidence required for contract monitoring.
* Demonstrate outstanding customer service; build sustaining relationships with service users, customers and other agencies; possess excellent interpersonal and problem-solving skills; show planning and organising ability; be able to use IT systems and maintain accurate and timely case notes.
Requirements and Qualifications
* Minimum of NVQ4 Advice and Guidance or a willingness to work towards this qualification after conclusion of a successful probation period.
* Knowledge and demonstration of safeguarding, health and safety, confidentiality and equality of opportunity; ability to apply these areas in practice.
* A full, valid car licence and access to a car with business insurance cover would be an advantage.
Benefits
* Salary £27,615 (full time 37 hours) for those with a relevant NVQ4 or equivalent; trainee salary scale £23,492 and £24,399 for those with a relevant Level 3 qualification or equivalent.

Safeguarding and Vetting
The post will be subject to an enhanced DBS check and prison vetting.
