We are a forward thinking Practice comprising of a large clinical team. The role is exciting as we have a large patient list size and nine GP Partners. You will be supported by an experienced team alongside a dedicated Practice Manager.

Job responsibilities
* Deliver practice nursing services independently and in partnership with the Clinical Team, adhering to the NMC Code of Conduct.
* Conduct patient assessment, treatment, screening, health education, and advice.
* Develop and implement individual treatment plans for long‑term conditions such as Diabetes, Cardiovascular Disease, Respiratory Conditions.
* Collaborate with other health professionals to monitor, manage and treat long‑term conditions.
* Review medication for therapeutic effectiveness and adherence to evidence‑based practice.
* Support patients with health promotion strategies and self‑care principles.
* Provide contraceptive and cervical cytology support to women, and advise on breast self‑awareness.
* Offer peri‑ and post‑menopausal symptom guidance with appropriate referral.
* Administer vaccinations & immunisations for adults and children, handling storage and disposal per policy.
* Provide opportunistic screening, wound care and smoking cessation advice.
* Provide advice on family planning and sexual health for men.
* Maintain accurate and contemporaneous records, ensuring data governance and confidentiality.
* Attend clinical and nurse meetings, contribute to shared learning and project work.
* Work within guidelines for vulnerable adults, substance abuse, and safeguarding.
* Collaborate with the manager on new initiatives, national guidance, policies and protocols.
Qualifications – Essential
* NMC Registration.
* General Practice experience.
* Chronic Disease Management experience.
* Proficiency in EMIS.
* Experience working in a GP Surgery or Primary Care setting.
* Training and experience in chronic disease management.
Qualifications – Desirable
* Qualifications in Long Term Conditions (Cardiovascular, Diabetes, Respiratory).
* Accredited training in chronic disease management.
* Knowledge of national guidance relating to chronic disease management.
* Strong communication and interpersonal skills.
* Effective time‑management and organisational skills.
* High standard of record keeping and clinical documentation.

* Disclosure and Barring Service Check required.
* Subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975.
* UK professional registration required.
