What You'll Be Doing:
Join our team as a Band 6 Social Worker and play a key role in supporting the older adults with mental health needs across three integrated teams in Bromley. In this role, you will:
* perform comprehensive patient assessments
* Caseload holding in a specialist older adult mental health team
* create and implement personalised care plans
* deliver evidence-based interventions to optimise outcomes
* collaborate with multidisciplinary teams to provide holistic care
* maintain accurate patient records
Requirements & Qualifications:
To be successful in this role, you will need:
* a current and valid SWE registration
* experience working with adults or older adults in mental health services
* strong communication and care planning skills
* the ability to work independently and within a multidisciplinary team
* confidence working in a fast-paced and integrated setting
Working Hours & Contract Duration:
This flexible working role requires availability from Monday to Friday, with part-time options considered. The contract is for 3 months with a strong possibility of extension based on performance and service needs.
