The Community Resource Team (CRT) is a large team, consisting of several disciplines, e.g., Social Workers, Occupational Therapists, Physios, Domiciliary Care staff. The Hospital Discharge element of the CRT is a fast-paced environment and the post holder will be vital in providing support to enable safe discharges, focusing on working with individuals to achieve their outcomes. The Hospital Discharge part of the service is made up of experienced and newly qualified Social Workers and Social Work Assistants who offer support to each other and are also supported by experienced Managers.
Responsibilities
* Provide support to enable safe hospital discharges and work with individuals to achieve their outcomes.
* Collaborate with a multidisciplinary team across the Community Resource Team to promote partnership working practices.
Qualifications
* A Diploma / Degree in Social Work or predecessor qualification.
* The ability to work constructively with other professionals to promote partnership working practices.
* Experience of working within the Continuing Health Care framework.
* Full UK Category B (Cars) driving licence and the use of a motor vehicle insured for business purposes to travel throughout the borough to visit clients and attend meetings.
