What the job involves As a team member of Community Health and Social Care, Seven Kings Cluster (HASS Seven Kings), you will work within a multidisciplinary team that includes LD Health practitioners such as physiotherapists, occupational therapists, psychologists, psychiatrists, speech and language therapists, community nurses, and administrators. Your role will involve managing a caseload reflecting diverse needs, delivering face-to-face assessments and reviews, and developing support plans. You'll work in a hybrid system—2/3 days in the office and the rest remotely—and will be office-based for a whole week every 4/5 weeks during duty weeks. Support, training, and supervision are provided, along with a laptop and smartphone for flexible work arrangements.
