Herefordshire Council’s Independent Living Service is looking for skilled, motivated HCPC-registered Occupational Therapists to join a supportive, forward-thinking Adult Social Care team. This is a role where your professional judgement matters, your time with people counts, and your work genuinely changes lives.
About the role
As an Occupational Therapist in our Independent Living Service, you will work with adults of all ages whose illness, disability or life changes affect their ability to manage everyday activities. You will support people to remain safe, independent and confident in their own homes and communities.
This is a community-based, prevention-focused role, grounded in strengths-based practice and early intervention. You will work autonomously while being fully supported by an experienced, friendly OT team.
What you’ll be doing
* Undertaking statutory Occupational Therapy assessments in people’s homes and local environments
* Completing functional, social and environmental assessments using a strengths-based, person-centred approach
* Identifying risks and barriers to independence and implementing preventative, enablement-focused solutions
* Recommending and facilitating:
o Aids and equipment
o Minor and major adaptations
o Moving & handling solutions
o Technology-enabled care
* Working collaboratively with:
o Social Care colleagues
o Health professionals
o Housing teams
o External agencies, carers and families
* Managing a varied caseload, prioritising effectively and making defensible professional decisions
* Supporting the development and supervision of junior staff and OT Assistants
* Maintaining high standards of professional practice, record keeping and legal compliance
How you’ll work
You will work in a flexible, agile way, either in a fixed, field-based or blended role. You’ll have the autonomy to manage your own workload while being part of a team that values reflection, learning and mutual support.
Our Occupational Therapy service is central to Adult Social Care prevention, helping to reduce, delay or avoid the escalation of care and support needs.
About you
You will be:
* A qualified Occupational Therapist, registered with the HCPC
* Experienced in community, adult or social care settings (or transferable relevant experience)
* Confident working autonomously and managing risk
* Skilled in communication, organisation and problem-solving
* Passionate about enablement, independence and person-centred practice
* Committed to continuous professional development and service improvement
