This is an exciting opportunity to join Dorset Council’s newly established Birth to Settled Adulthood Service, working directly with young people and their families as they transition into adulthood. This innovative service is designed to provide seamless support, ensuring young people’s needs are met through a well-planned and informed approach.
As part of this new team, you will play a key role in shaping and promoting the service, working collaboratively with colleagues across children’s and adults’ social care, health, SEND, and education. Your work will focus on preparation for adulthood for young people aged 14 to 25, including Care Act assessments, care and support planning, and Mental Capacity Act (MCA) assessments and Best Interest meetings.
